I think the opening scene of the movie is to show everything that is bright, serene and innocent and everything that is for show like the flowers in the first shot. I think the bugs represent the wildness and darkness of the underbelly of society. They represent the disease beneath. Disease could be that of the body like a heart attack or stroke or could be that of the soul being evil. They both can pull you into the darkness.
