No, that impressive gut is 100% real. You can see it, for instance, in the short film featuring Goaz that came out around the time of the return announcement (and features Goaz waiting for a call from Lynch). Greeting (or, better... HELL-OOOOOO!)
I lurked for the past couple of days.
Here's my take:
1) The more the story goes, the more it becomes coherent. Watching the 1st episode was a continuous WTF. The beginning of E3 was WTF but mainly for what was on screen, not plotwise. It was clear that Dale was moving towards the various planes. At the end of S4 things are getting a little bit clearer (yet weird).
2) Wally Brando was great! At first I was like "when does this scene ends?" And then I was on board with him. It was also a clear trolling session by Lynch.
3) Dougy!! I love him, but I can't wait to see him go.
4) The coffee scene is clear: things are going back to "normal". The real Cooper, and the old "coffee and cherry pie" Twin Peaks is there. We just have to wait a little bit more, after all we had our main character in the Black Lodge for 25 years...
5) What is watching over Cooper? He was aided both in the Black Lodge, and at the Casino. Also the owl was looking down on him when he stood in front of the red door, he was aided from being hit by the snipers etc. So what is watching after him? And why was he let winning jackpots over jackpots?
I am throughly impressed by this serious.

I think that cgi was only used to create unnatural effects occurring in the red room (such as movement of the Arm and the pullback of the drapes to reveal the white mare), but not to create the red room itself. It's much easier to create a real life replica of the red room then to recreate the room digitally. The setting of the red room is generally simple and Does not involve the level of detail that the Star Wars prequels used for their backgrounds.
It seems like it's the use of HD, rather than film like the original series, which is creating an artificial artiface.
As a non-sequitar, I rewatched the scene with the black and white tiles moving up and down when the Arm's doppelgänger corners Cooper, and it looks like that scene was created using practical and not digital effects.

After a second re-watch, I've gone from Cooper is still in a lodge/ alternative dimension Mulholland Drive style world to that he is indeed out in the "real" world. All that said, am I the only one who gets a distinct Naomi Watts pre and post MD shift with the 1-9-9 mother and son? Something about the kid especially seemed very directly alternate Sonny Jim. Also, that was a Heather Graham look alike in the casino, but maybe randomly so.
And I'm just assuming the Lost Vegas scene in the pilot where the casino owner gives money for someone to do a job is him putting out the hit on Dougie Jones, based on orders from Evil Cooper, who somehow through interaction with the dancing dwarf/ swaying tree has created a clone of Cooper to give him a loop-hole out of being sucked back to the Lodge (hell?) and knew aproximately when and where to put out the hit on Dougie/ Coop. On the other hand, I theorize that Evil Coop was deliberately putting himself in federal prison (based on him scanning the plans), and am not sure how all this works with the timing of the car crash and Dougie being sucked into the Lodge.
Lastly, one episode back, but is the obvious thing in that whole New York staring into the box sequence that it is about coffee? Whatever is breaking through that box is going for something, and those two cups of coffee look really enticing, given that the show spends about 10 minutes establishing that the coffee is there... Maybe it really is Coop in some form or another coming through?

I thought this too -- it's a frustratingly near-perfect anagram of "Waldo Mynah Bird," but it's off enough that I don't think it's intentional (or else is poorly done if intended).
