The masked figure goes back to Episode 2, where he was probably not intended to be Leland (and, notably, was not in the script...an on-set improvisation by Lynch perhaps?). Somebody posted a Q&A from the Long Beach Comic Con on YouTube. Around the 35-minute mark, the cast members are asked about a possible fourth season, and about the Emmy snubs. They don't really address the fourth season but Kimmy, Sherilyn, and others indicate they blame Showtime for the absence...

I imagine the idea for the tensions between Ben and his assistant only came up after LFB declined to appear, and Beverly was created to fill Donna's role and give Ben somebody to interact with in that office of his. The finished screenplay was said to be 525 pages. I'm not sure if that refers to the total pages of the script on the day Lynch started filming in Sept 2015, or if it's the final count of the final shooting screenplay (at the conclusion of filming in April 2016).

A few notes on things that were confirmed in the book Room to Dream that I thought would be worth adding to this thread: Lynch and Frost submitted their script of the full (or then-full) limited series to Showtime in January 2015. That script was 334 pages. Lynch continued to write additional scenes...

It was definitely a then-timely joke about President Trump's loose lips. They gave that interview the day of breaking news that Donald Trump revealed highly-classified information to the Russian foreign minister and ambassador when they were visiting the White House.

I found it funny to read that the Green Glove was originally an idea Lynch had for Jack Nance!
